1 Purpose
To prevent and reduce safety accidents, personnel casualties, or economic property losses during the service activities of surveying and design (engineering consulting), general contracting, and logistical support production, and to effectively improve management methods and control measures, this procedure is specially formulated.
2 Scope of Application
This program applies to safety accidents (incidents) or potential unsafe factors and non-compliance corrections and preventive controls occurring in various units under the group company.

3.3 "Regulations on Emergency Rescue and Investigation of Railway Traffic Accidents" State Council Order (No. 501) amended according to the State Council Order No. 628 of the People's Republic of China published on November 9, 2012, and implemented from January 1, 2013, in accordance with the "Decision of the State Council on Amending and Abolishing Certain Administrative Regulations."
3.4 "Safety Production Management Measures of China Railway Fourth Survey and Design Institute Group Co., Ltd." (Fourth Institute Safety Quality [2017] No. 749)
4 Definitions
4.1 Safety incidents: Accidental events that result in death, injury, illness, property loss, or other losses.
4.2 Safety incidents: Situations that may or have caused injury and health damage arising from work or occurring during the work process.
4.3 Acute poisoning incidents: Refers to the exposure to production-related toxic substances through the respiratory tract or ingestion in a single instance or over a short period.
Chemical exposure or skin contact can lead to significant internal absorption, causing rapid bodily changes, resulting in work interruption, requiring emergency treatment, and even death.
4.4 Non-compliance: Any actions or deviations that can directly or indirectly cause personal injury, illness, property damage, or damage to the work environment, and that violate operational standards, practices, procedures, or management system requirements.
4.5 Busy trunk lines; refers to the Beijing-Harbin (excluding the Shenyang-Shanhaiguan line), Beijing-Shanghai, Beijing-Guangzhou, Beijing-Kowloon (including the section from Guangzhou to Shenzhen), Longhai, and Shanghai-Kunming (excluding the section from Zhuzhou to Kunming) lines, as well as passenger dedicated lines. 4.6 Secondary units: production units, business divisions, auxiliary production units, logistics support units, project departments, subsidiaries
5 Responsibilities
5.1 Chairman and Dean: Responsible for the overall safety production work of the group company. Responsible for setting up safety management organizations, establishing management systems, providing resource guarantees, and developing emergency response plans.
5.2 Group company leaders in charge: Responsible for the organizational leadership of safety production within their scope of work, approving corrective and preventive measures for major incidents and non-conformities, and organizing rescue and handling of major safety accidents that occur.
5.3 Administrative Office; responsible for organizing the handling of major safety accidents and security incidents that occur in local delegations and the group company. Based on the analysis of the causes of non-compliance with safety accidents (incidents), organize the identification and assessment of non-compliance, and propose corrective and preventive measures, as well as suggestions for systematic improvements to the responsible leaders. Supervise each unit to promptly address and implement checks on major incidents and non-compliance.
5.4 Quality and Safety Department: Responsible for organizing the handling of major safety incidents related to production, fire, and traffic that occur under local commissions and the group company. Organize the identification and review of non-compliance, and propose corrective and preventive measures, suggesting system improvements to the responsible dean. Supervise each unit to promptly address and implement checks on major events and non-compliance.
5.5 Technical Center: Participate in the investigation of safety and quality accidents in engineering construction caused by design quality issues. Based on the analysis of the causes of the accidents (incidents), provide improvement suggestions to the group company.
5.6 International Department: Reasons for Non-compliance with Safety Incidents (Events) in Overseas Projects of the Group Company
Analysis and accident response organization work.
5.7 Secondary Units: Responsible for the identification, review, and control of safety incidents (events) that do not comply within their unit; and for investigating and analyzing incidents (events) that occur within their unit that do not comply. According to the requirements of the group company, they will handle the responsible parties for the incidents and formulate corrective and preventive measures. The investigation and handling reports of non-compliance incidents (events) will be submitted to the Safety and Quality Department, Technical Center, and Administrative Office by category.
5.8 Accident Investigation Team
5.8.1 If an investigation is organized by higher authorities, local governments, or industry regulatory departments, relevant units of the group company should cooperate effectively.
5.8.2 If the investigation is entrusted to the group company, the investigation team should ascertain the sequence of events, causes, casualties, damage to equipment and facilities, and economic losses. They should also carry out the following tasks:
a. Determine the nature of the accident, identify the responsible units and personnel, provide the basis for the determination, and suggest measures for dealing with those responsible.
b. Clarify the lessons learned from the incident and propose corrective and preventive measures.
c. Write the accident investigation report.
6 Work Procedures
6.1 Safety incident events do not conform to the management flowchart.

6.2 Classification and Levels of Safety Incidents
6.2.1 Production and Railway Traffic Safety Accidents
a. Particularly serious accident
Causing the death of more than 30 people, or serious injury to more than 100 people (including acute industrial poisoning, the same below), or direct economic losses of more than 100 million yuan.
A busy mainline passenger train derailed with more than 18 cars, interrupting railway operations for over 48 hours.
A busy freight train derailed with more than 60 cars, interrupting railway operations for over 48 hours.
b. Major accident
Causing the death of more than 10 but less than 30 people, or serious injury to more than 50 but less than 100 people, or direct economic losses of more than 50 million yuan but less than 100 million yuan.
More than 18 passenger train cars derailed.
A freight train derailed with more than 60 cars.
Passenger trains derail with more than 2 but fewer than 18 cars, interrupting operations on busy mainline railways for more than 24 hours or interrupting operations on other rail lines for more than 48 hours.
A freight train derailed with more than 6 but fewer than 60 cars, interrupting operations on busy mainline railways for over 24 hours or interrupting operations on other lines for over 48 hours.
c. Major accidents
Causing the death of more than 3 but less than 10 people, or serious injury to more than 10 but less than 50 people, or direct economic losses of more than 10 million yuan but less than 50 million yuan.

6.2.2 Classification of Fire Accidents
a. Major fire accident: a fire that causes more than 30 deaths, more than 100 serious injuries, or direct property losses exceeding 100 million yuan.
b. Major fire accidents: fires that result in the death of more than 10 but fewer than 30 people, or serious injuries to more than 50 but fewer than 100 people, or direct property losses of more than 50 million yuan but less than 100 million yuan.
c. Major fire accidents: causing the death of more than 3 but fewer than 10 people, or serious injuries to more than 10 but fewer than 50 people, or direct property losses of more than 10 million yuan but less than 50 million yuan.
d. General fire accidents: fires that result in the death of fewer than 3 people, or serious injuries to fewer than 10 people, or direct property losses of less than 10 million yuan.
6.2.3 Classification of Road Traffic Accidents
a. Particularly major accidents: incidents that result in the death of 3 or more people, or serious injuries to 11 or more people, or the death of 1 person along with serious injuries to 8 or more people, or the death of 2 people along with serious injuries to 5 or more people, or property damage exceeding 60,000 yuan.
b. Major accident: An incident that results in 1 death, 2 people injured, or 3 to 10 people seriously injured, or property damage of more than 30,000 yuan but less than 60,000 yuan.
c. General accidents: incidents that result in serious injuries to 1 to 2 people, or minor injuries to more than 3 people, or property damage of less than 30,000 yuan.
d. Minor accidents: incidents resulting in minor injuries to 1 to 2 people, or property damage in motor vehicle accidents of less than 1000 yuan, and non-motor vehicle accidents of less than 200 yuan, totaling four levels.
In the data above, "above" includes the number itself, while "below" does not include the number itself.
6.3 Accident Report
6.3.1 In the event of an accident at a unit belonging to the group company, the parties involved or the on-site person in charge should immediately report to the head of the unit. In urgent situations, they may report directly to the relevant local government departments (the report should include: the time and place of the accident, the circumstances, casualties and losses, and the emergency measures taken, etc.). If the accident results in injuries, immediate assistance should be sought from the local medical emergency department.
6.3.2 After the person in charge of the accident unit receives the report, they shall immediately report to the group company (production, transportation,

6.3.3 After the competent department receives the accident report, it shall immediately report to the dean and the responsible leaders of the institute. After obtaining approval from the group company leaders, it shall report to China Railway Construction Corporation Limited and the local government or relevant departments responsible for safety supervision within 1 hour.
6.3.4 Units that experience a safety accident should actively take effective measures to fully rescue the injured, protect the scene, and actively organize the aftermath handling work.
6.4 Accident Investigation
6.4.1 Composition of the Accident Investigation Team
a. The higher-level and local governments are responsible for the investigation and handling, and the investigation team is composed of both higher-level and local officials; the group company will assist in related work.
b. The entrusted group company is responsible for the investigation and handling, led by the responsible 院领导 as the team leader, with heads of relevant departments such as safety and quality, technology, supervision, and the labor union participating in the investigation team.
In the case of minor accidents resulting in direct economic losses of less than 30,000 yuan without personal injury, the grassroots unit (team, office, institute) is responsible, with the unit leader serving as the team leader and relevant personnel participating to form the investigation team.
In the case of a general accident resulting in minor injuries and direct economic losses of less than 30,000 yuan, the secondary unit is responsible, with the unit leader serving as the team leader and relevant personnel participating in the investigation team.
For major accidents resulting in serious injuries and direct economic losses exceeding 30,000 yuan, the group company is responsible, with the designated leader from the relevant department, and heads of safety, quality, technology, supervision, and union participating in the investigation team.
In the event of personnel deaths and major traffic accidents (direct economic losses exceeding 300,000 yuan), an investigation team will be formed by the higher-level and local governments and relevant departments, with the heads of the group's safety, quality, technology, supervision, and union departments cooperating with the investigation team.
6.4.2 Accident Investigation Team Work Procedures
On-site handling: record the original condition, draw a site diagram, collect accident evidence.
b. Collecting information: Investigate the circumstances and causes of the accident, record witness testimonies, gather relevant regulations, process flow documents, and injury diagnosis certificates, and organize a technical assessment of the collected information.
c. Accident analysis: Identify the type and cause of the accident, clarify the responsible parties, and propose handling suggestions.
d. Formulate corrective measures: strengthen safety education, improve process conditions and working environment, and address weak links and defects in management.
e. Investigation Report: After the on-site accident handling is completed, the accident investigation team will complete the accident investigation work within three days, write the accident report, submit it to the superior and the entrusted department, and file it with the Safety and Quality Department or the Technical Center and other relevant departments.
6.5 Accident Handling
For accidents of level 6.5.1 and above that have already been addressed by higher authorities and local governments, execute according to the approval opinions issued by the higher authorities and local government.
6.5.2 Handled by the group company shall be processed in accordance with the following provisions.
a. For direct economic losses below 30,000 yuan without personnel injuries, the grassroots units (teams, offices, institutes) are responsible for handling and must report the results to the group company.
If there are personnel with minor injuries and direct economic losses below 30,000 yuan, the secondary unit is responsible for handling it and must report the handling results to the group company.
If there are personnel with serious injuries and direct economic losses exceeding 30,000 yuan, the group company will be responsible for handling it.
6.6 Handling of Events and Non-Conformities
6.6.1 During the production, service, and activity processes, any non-conformities and incidents identified through performance monitoring by each unit shall be promptly addressed and corrected by the responsible unit to prevent the escalation of incidents into accidents.
6.6.2 For significant events or serious non-conformities discovered, the reasons should be carefully analyzed. The responsible unit should take effective control measures and immediately report to the supervising leader and the secondary unit, with corrective actions taken by the responsible unit. The implementation of corrective and preventive measures should be tracked and verified, and the results of the corrections should be reported to the Safety and Quality Department.
6.6.3 The Quality and Safety Department, Technical Center, and Administrative Office are responsible for the regular evaluation of the group's safety performance and for writing evaluation reports periodically, which are submitted to the responsible leaders of the institute.
6.6.4 Non-conformities identified during internal and external audits shall be handled in accordance with QESP901 "Internal Audit Management Procedures" and external audit requirements.
6.6.5 The responsible 院领导 reviews the handling of major incidents and serious non-conformities, and deploys work to resolve major incidents and serious non-conformities.
