UNDERGRADUATE MIDTERM EXAM/FINAL MAKE-UP EXAM REQUEST PETITION

I could not attend the midterm/final exam of my course, the information of which I have given below, due to my excuse.
I respectfully submit to your information that I need to participate in the make-up exam of the course in question.
Note: THIS FORM MUST BE FILLED OUT AGAIN for each exam not taken during the reporting period.
"ARTICLE 31 - (1) Students who cannot take their exams for just and valid reasons are given the right to take a make-up exam to be used on the day and time to be determined by the instructor who teaches the course in the same semester and with the decision of the faculty administrative board. In order for students to take the make-up exam, they must submit their excuse petitions to the dean's office of the faculty they are affiliated with within 5 working days from the date of the exam.
The acceptance of the excuse is ensured by the submission of just and valid reasons, together with the proving documents.
In order for the student to be considered disabled with a health report, he/she must document his/her illness with a health report from health institutions. The grades obtained by the students in the exams they take during the reporting period are deemed invalid.
If the student has to interrupt his education due to the extraordinary natural events in the place where the student's family resides, or if he cannot take the exams, he must document it with a document to be obtained from the provincial or district administrator where the unusual natural event took place.
In case of serious illness or death of the student's first-degree relatives, a health board report is requested in case of illness, and an official document is requested in case of death.
In terms of the content and consequences of the judiciary, the student is deemed to be excused in case of temporary detention that does not remove the student's right to be a student according to the relevant disciplinary regulation or does not require the removal of this right.
Students who have the right to be a student in any double major or minor program in addition to their major are deemed to be excused for their exams in the double major / minor program if their exams in the major program and the exams in the double major / minor program coincide on the same day (REVERSE DOES NOT APPLY).For this reason, students who certify the exam day and time in the major program with the approval of the relevant department chair are excused from the exams with the same date and time in the double major/minor program.
With the scientific, cultural, social and sports activities of our university, students who represent our country at national and international level at home and abroad reserve the right to midterm and / or semester / final exams.
The student is deemed to be excused for other reasons that the relevant board of directors will deem justified and valid.

Example-1: If 1 day is written in the content of the health report you received from the hospital;

As seen in the example above, 1 day is selected on the same day for the Excuse Start and End dates in the reports.
Example-2: If 2 days are written in the content of the health report you received from the hospital;
